A quote request form helps a potential customer explain a job so you can prepare a relevant estimate or proposal. Use these free quote form templates to collect scope, timing, and supporting details before the first pricing conversation.
Marketing quote form: campaign goals, channels, deliverables, and the split between service fees and advertising spend.
Freelance quote form: a project brief, the expected output, review needs, and deadline.
Electrical quote form: work requested, property context, access, and whether an assessment is needed.
Translation quote form: source and target languages, document size, format, and delivery date.
What should a get-a-quote form ask?
Keep the questions that change how you estimate the work. Here is a starting set to adapt to your service:
What do you need help with, and what would a successful result look like?
What size, quantity, deliverables, or features are involved?
What is already available, and what would you need us to provide?
When is the work needed? Is that a fixed deadline or a preference?
Are there site conditions, access needs, or other requirements we should review?
Do you have a budget range, or would you like help scoping the options?
What is the best way to reply about this request?
Add an optional file upload when a brief, photo, or drawing would help. Explain what to send and offer a way to continue if the customer does not have it yet.
Example: turn a website request into a quotable scope
Imagine a customer asking for a new website with a blog and a launch deadline. The page count alone does not tell you the work involved. Ask whether the content is ready, whether existing posts need moving, and whether the launch depends on another event.
If they need copywriting, show a follow-up about the content they already have. If they need migration, ask about the current site and what must move. For an online store, ask which products and store operations are in scope before giving an estimate. These are example questions to configure in your copy, not a promise about the template’s preset logic.
Use the answers to separate design, development, content, migration, and ongoing support in the proposal. Confirm assumptions, review rounds, and exclusions with the customer before agreeing the price.
Set up the request and follow-up in Youform
Make your copy. Open a template and choose Use this template. Replace the sample service and questions with your own.
Ask relevant follow-ups. Add conditional logic for details that depend on the selected service. Keep the first request focused enough for a prospect to finish.
Choose where requests arrive. Use an email notification to yourself, Google Sheets, or a configured automation. Assign someone to review new requests.
Explain the next step. Tell customers whether you will reply with an estimate, ask for more detail, or arrange an assessment. Give a reply time your team can meet.
Check and share. Review the wording, required questions, conditional paths, and phone layout before placing the form on your website.
A request, an estimate, and an order are different steps
These templates collect a request. They do not automatically generate a final quotation, invoice, or confirmed order. Youform supports calculations, but you must configure and maintain suitable pricing rules if you choose to show an estimate.
Keep a review step when site conditions, scope, materials, availability, or other unknowns can change the price. Show what an estimate includes and explain what still needs confirmation. Use your quoting and order process to agree the final details.

Frequently asked questions
Does a quote form calculate a final price automatically?
No. These templates collect the information you need to prepare a quote. You can configure calculation rules, but you must maintain the prices and assumptions and check whether an assessment or review is needed before confirming a price.
Are these quote request form templates free?
Yes. Make a copy with a free Youform account and customize the questions. Free includes logic, website embeds, file uploads up to 10MB each, and unlimited responses within fair use. Custom domains, branding removal, and payments require Pro. See current plans.
Can I put a quote request form on my website?
Yes. Share the form link or use Youform’s website embed. Explain the service and follow-up process near the form. A website embed does not automatically create quotes or add requests to your CRM; configure that workflow separately.
Should I ask for a budget on a quote request?
Ask when it helps suggest a suitable scope or package. Offer ranges and an undecided option when customers may still be exploring. Budget does not replace the project details needed to estimate the work.
Does submitting the request confirm an order?
No. A request starts the conversation. Review the scope, price, availability, and terms through your own quoting process before confirming an order.
